This 25 bed Critical Access Hospital is seeking a Case Management Manager to provide leadership to the staff and oversee daily functions of the Case Management department. Responsibilities include utilization review; care coordination and resource management based on patient assessment and needs after discharge or transfer. The manger ensures the department is following CMS Conditions of Participation as well as TJC’s Provision of Care. The Case Management department interfaces with all departments at the Medical Center. 

• This position requires an associate degree in a related field, current Ga RN license, AHA BLS, 2 years’ experience in Case Management and 2 years progressive leadership experience.
• Reports to: Director of Revenue Cycle
• FTE’s: 3
