Ortho Case Management opening in NC

A newer hospital in NC has a need for an Orthopedic Case Manager to join their growing team.

 

 
This role will be expected to support the needs of the orthopedic program from pre-hospital to post-hospital encompassing the entire continuum of care.

 

 

 

 

The Nurse Case Manager is a Registered Nurse who is responsible for providing patient case management services to assigned populations.

 

 

 

 

He/she utilizes a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the patient’s plan of care, discharge plan, and re-entry into the community.

 

 

 

The goal of his/her intervention is to meet the needs of patients in order to optimize clinical and resource outcomes, and to monitor the effectiveness of the plan and its outcomes.

 

 

 

In collaboration with the social worker, the nurse case manager identifies at risk patient populations who require case management services.

 

 

 

He/she provides bio-psychosocial assessments to identify patient needs and to assist in the development of the plan of care.

 

 

 

The nurse case manager serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, community resources, and payers to reduce redundancy and fragmentation of care.

 

 

 

He/She anticipates potential delays in the health care process and acts proactively to avoid these delays.

 

 

The nurse case manager ensures that necessary services are provided at the most appropriate level of care and that there is a smooth progression of the patient throughout the health care system and into the community.

 

 

 

Position reports to Dir, Women’s Services and Med Dir, Case & Clinical Resource Management.

 

 

Experience:

 

 

 

Three years of recent clinical experience required.

 

 

 

Five years of relevant RN experience preferred.

 

 

 

Additional experience in case management, discharge planning, third party review, and/or home health care preferred.

 

 

 

Education:

 

 

 

Registered Nurse and baccalaureate required.

 

 

 

Masters preferred.

 

 

 

Licensure/Credentials:

 

 

 

Current license as an RN or valid Status P (Petitioner) temporary license in State of North Carolina with no substantiated findings required.

 

 

 

Status P indicates RN has applied for permanent NC license.

 

 

 

ACM or CCM certification required within one year of eligibility.

 

 

Hours of Work 7:30-6pm 4 days/week
