Director of Care Coordination job opening located in Northern California
Director of Care Coordination:
This is a 90-bed hospital (actual beds are 130-take away 16 for Mother Baby and another 24 for ICU). Level 2 Trauma Center. It is not certified as a stroke center, but they see extremely complex stroke cases. The trauma and neuro cases tend to be longer stays. They have been as high as 5.0 days per patient, but they want to be closer to 4.2 or lower.
They need someone who is front and center. They can have experience as a social worker or a Case Manager. They are not looking for a desk jockey. Someone who can round (complex cases) and be a member of the team. The entire team (from the CEO down) is very hands-on. If she (the CEO) needs to call an LTAC CEO to make the case for a patient to be moved, she will. She will make things happen. She will intervene. She will help, she welcomes it. Whoever takes the position, will have complete support from the team, including her. Utilization Management is also very involved.
They are looking for someone who is energetic and has a sense of urgency. Someone who has good problem-solving skills and has creative ideas. They must have experience in management.
There is a manager in place and about 30 staff made up of case managers and social workers.
