Job openings for 3 Interim Case Managers located in 3 different medical centers in Indiana, northeast of Indianapolis.
3 case managers – One could possibly work remote after spending a week at the hospital.
The assignment – 12 week assignment that may be extended.
Looking for experienced staff RN CM with 2 to 5 years recent acute care discharge planning hospital experience to work on the weekends for 12 week assignment with temp to perm possibility. They will need to work Thursday through Monday with focus on performing discharge planning assessments and referrals. Preference is to have CM RN staff to work temp to perm 12 week assignment in Muncie Indiana on site however will interview staff who can come in for one week on site training and perform rest of initial review and referrals remotely. Our Covid census has recently decreased so onsite may be more doable than in the past.
- Must have compact RN license to work in Indiana
- Acute care case management discharge planning experience a must
- 40 hours per week either 8 or 10 hour days upon manager approval
- Prefer BSN
- Experience with Cerner CM module a plus
This job requires staff RN CM with recent CM hospital experience who can hit the ground running with exceptional assessment and communication skills as well as ability to document all referrals initiated.
Anyone working on or off site needs to clear employee health list of tests and vaccines needed.
