A health system in the Raleigh, NC area, has a need for an experienced Clinical Operations Director for Public Health, to join their team. Ideally, they would like candidates to have experience with Case Management and Public Health/ACO experience. They would consider nurse directors as well.
Job Summary:
The Director, Clinical Operations for Public Health, provides leadership, oversight and strategic direction to a coordinated Population Health Management care model that spans the continuum of care designed to improve quality, reduce cost, and promote patient and provider satisfaction. Partner with key physician and practice stakeholders in setting the strategy for an effective continuum-based care model including but not limited to: Care Transitions, Complex Case Management, Condition Management, Care Coordination and Community Based Resources.

Education:
- Master’s Degree Health Administration Or Nursing Or Public Health Required
Experience:
- 3 Years Management – Direct Area of Responsibility Required –
- And Clinical – Case Management And Supervisory Preferred
Licensure/Credentials:
- Registered Nurse Required
