300+ bed hospital in GA, has a need for an experienced Diabetes Educator to join their team.
This position will be working with adults.
The job requires a RN and the CDE certification is preferred.
Coordinates and manages the group and individualized, goal-directed diabetes education within the standards set by the hospital directives and by the American Diabetes Association and the American Association of Diabetes Educators.
Provides support and intervention to promote diabetes management for patient care.
Facilitates and processes patient documentation and physician referrals to coordinate outpatient diabetes self-management service provision.
Coordinates the on-going patient care directives specific to verification of diabetes education provision and interventions to support nursing staff and patient disease understanding and metabolic control.
Coordinates with physicians, personal, and staff to ensure quality diabetes self-management services and critical documentation are consistently provided.
Demonstrates the skills and judgment necessary to implement full diabetes self-management education and clinical intervention consistent with published standards of care in a manner that recognizes patient individual acceptance, cultural/religious or psycho social concerns.
Coordinates the on-going verification of ADA (American Diabetes Association) program standards to support maintained full ADA program certification.
Directly supports the coordination of the Outpatient Diabetes Management Program and markets diabetes and wellness specific community offerings.
Maintains up to date and accurate documentation of diabetes education/ care provided to ensure the integration of information for use by the health care team to ensure quality care.
Supports through professional and clinical practices the needs and objectives of St. Francis Hospital in regards to patient and staff interactions.
Functions independently in the work environment and as a role model for current and new staff.
Promotes effective working relations and works effectively as part of a department/unit team inter- and interdepartmental to facilitate the department’s/unit’s ability to meet its goals and objectives.
Other duties as deemed necessary.
JOB REQUIREMENTS:
Minimum Education:
Bachelor of Science degree in Nursing preferred
Significant clinical knowledge of diabetes care
Minimum Work Experience:
Two years of clinical direct patient care experience
Demonstration understanding of diabetes education principals and diabetes specific considerations of differing populations and life cycle stages
License(s) / Certification(s):
Current state license as applicable: Registered Nurse
Certified Diabetes Educator (CDE) preferred
CPR Certification
