Experience: Minimum of two (2) years of case management/care coordination experience to include leadership and management experience. Minimum of three (3) years acute care experience to include experience in discharge planning procedures. Medicare/Medicaid guidelines/knowledge of community resources
Summary:
Provide leadership for the case management team and oversee the hospital Case Management Department’s daily activities. Coordinates the integration of Social Services/Case Management functions into the patient care, discharge, and home care planning processes with other hospital departments, external service organizations, agencies, and health care facilities. Conducts concurrent medical record review using specific indicators and criteria as approved by Medical Staff, JCAHO, CMS, and other State agencies. Acts as a patient advocate, investigates, and reports adverse occurrences, performs staff education related to resource utilization, discharge planning and psychosocial aspects of healthcare delivery.
Directs and coordinates the integration of Social Services functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.
Facilitates and coordinates hospital-wide discharge planning and support programs to ensure compliance with Joint Commission and other health care accrediting standards in order to enhance the quality of patient care consistent with hospital policies for the adult and geriatric patients, 18 years and over.
Education: Bachelor’s degree in Nursing or related field preferred. Experience with critical care, trauma, or other specialty setting preferred. Marketing experience preferred. Ability to effectively communicate with all levels of hospital staff, physicians, and patient referral sources a must. Master level Social Worker Degree preferred.
Licenses/Certification: Currently licensed Registered Nurse in the D.C. preferred.
Current Basic Life Support (BLS) Certification LICSW or LGSW preferred.
