Director of Case Management job opening located in New Hampshire.
Base salary plus quarterly bonus.
Position is included in the hospital incentive plan. Hospital has been successful in hitting targets for bonus each quarter, hit 3 out of 4 last year.
CCM or ACM certification not required to start, but want candidate to obtain certification within the first year, hospital will pay for certification.
Need to be licensed if RN, Social Worker or Therapist.
Open to either RN, licensed social worker or therapist (PT, OT, SLP) – must have acute care case management, discharge planning, and utilization review experience plus some leadership experience (manager or above).
Total direct reports: 5 total (4 case managers, 1 case manager assistant); 4 case managers are 2 RN and 2 social workers.
Hospital ADC is 44; Total beds is 50 and stays pretty full
Typical case load expected of the case managers is 11-13
There are times when DCM takes a case load and has to come in after hours or weekends; DCM does on-call weekend (Sr. Team rotates every 8-10 weeks, call doesn’t start immediately for new DCM).
Interview process:
1st interview with CEO and HR Director (if local in person, if not via Teams)
2nd interview with Sr. Team onsite
3rd interview with Regional DCM via phone
