Market Director Case Management job opening located in the Central Valley of California.

Fresno area or nearby would be ideal. Needs to live somewhere near these facilities within commuting distance.

This is a Market Director spot to oversee 4 campuses, one is about 250 beds the others are under 80 beds. Will support the hospitals in Hanford, Reedley, Tulare, and Selma. 10% bonus and relocation. Director/Manager Case Management at each campus.

RN with BSN or a MSW or LCSW required.

Need about 5 years in Case Management leadership.

Position overview:

Director, Case Management: Direct function and personnel of the care management department. Develops, manages, and oversees the annual care management budget. Prepares and evaluates monthly, quarterly, and annual reports of the department’s functions. Monitors and reports on changes in Medicare regulations and documentation issues to physicians and others as needed. Maintains relationships and contractual oversight with key stakeholders, local, state, and federal agencies. 

Exempt Job Requirements: Education and Work Experience: Bachelor’s Degree or equivalent combination of education/related experience: Required. Master’s Degree: Preferred

Ideally seven years’ hospital discharge planning, utilization review, case management and social work experience: Preferred. Five years’ leadership experience.
