- Madison, NC, 170 beds, ADC 140, Administrator was there for 30 years and now a wonderful Mobile Administrator is there and has turned this facility around. Census is growing, agency staff almost gone, great Team spirit and Family – like work environment.
- Washington, NC, 150 beds, ADC 119. All other department managers in place except DON and ADON. Leadership struggling to remove agency staff. Leadership change will be good.
- Ayden, NC, 82 beds, ADC 77. New administrator recently resigned, after hours didn’t work with her Family. Also seeking a DON for this location. Using some agency staff, but facility is stable. Recent Complaint Survey November 12th, waiting 2567, but no high level scope or severity tags expected’Grantsboro, NC, 104, ADC is 65. Last Survey was in March with 9 low level deficiencies. Very stable facility. No agency staff. The administrator is leaving because of an opportunity closer to home.
- Jacksonville, NC, 246 beds, ADC 135. 2 IJ’s were passed non-compliance as we self-identified and corrected them. Stable management Team, sporadic agency usage on the weekends, census growth is a key focus, Facility is located across from a major hospital.
- Kernersville, NC. 92 beds, ADC 85. Stable Center. No survey issues, had a wonderful survey 3 months ago. Leadership team in place. Well run facility. Administrator just wanting a change.
- Mint Hill, NC, 120 beds, ADC is 82, Admissions Director has a pending candidate, and most recent survey is in process, with 17 low level citations. Great facility, one of our newer ones. Administrator is going back into retail.
