Southern California based Transitions of Care healthcare organization is looking for a NP to join their team.
They are a fairly new organization with a highly specialized offering – namely they are the FIRST and ONLY Transitions of Care organization that is approved by Medicare.
This means that patients in the hospital prior to discharge have the personal choice to opt in for post discharge care by a Nurse Practitioner for one month.
The role is special as it is totally independent.
Each NP makes their own schedule that involves visits and case management care of up to 20 discharged high risk patients at one time.
All patients are within a 10 mile radius of one another.
The NP can flex their schedule to meet their needs.
The role itself is less of hands on medical care and more of case management.
The NP visits the patient at home and orders wound care services if needed and the like.
The goal is to help the patient heal post discharge and avoid re-admittance to the hospital.
This NP would have the opportunity to also grow into a Lead NP role if they would like.
Competitive salary plus monthly car stipend.
No relocation provided as they want someone who lives in Southern CA.
