Director, Care Coordination, Patient Flow & Community Partnerships position. – will look at strong CM Director/Large hospital or System level candidates only.

Position Summary:
· Provides leadership in identifying and establishing an effective system of care coordination, patient flow, and bed utilization in all patient care areas; including an on-going process improvement plan to ensure effectiveness and efficiency of functions and services at all times.
· Responsible for developing and maintaining effective partnerships with related community institutions, agencies, organizations, and providers.
· Implements strategies to achieve optimal delivery of services of assigned areas.
· Formulates strategies to promote and establish effective partnerships in the community.
· Directs the management of functions, services, activities of assigned areas.
· Works closely with the management team, nursing, physicians and other disciplines.
· Maintains regulatory compliance, monitors results, and promotes cost effective protocols.
Qualifications:
Bachelor’s degree in Nursing.
Master’s degree in Nursing, Business Administration, Health Administration or related field.
Current Hawaii licensure as a Registered Nurse – will need to have prior to start date.
Experience:
Five (5) years management experience in an acute care setting, preferably in a comparable organization to the Medical Center.
Solid understanding of business principles, i.e., strategic and financial planning, budgeting, business planning, organization theory.
Proven leadership skills, i.e., planning, communications, motivation, mentoring, interpersonal skills.
Track record of managing multiple departments/functions, preferably in a comparable health care setting.

Telemetry Manager
Responsible for overall operations of a 40-bed Telemetry unit, 90+ FTEs in a 100+bed acute care hospital.
. Bachelor’s degree in Nursing.
. Master’s degree preferred.
. Current Hawaii license as Registered Nurse.
. Evidence of continuing education.
. Current BLS/HCP certification.
. Minimum five (5) years progressive experience as a Registered Nurse in an acute care facility of which at least three (3) years have been in a leadership capacity demonstrating the ability to manage assigned areas effectively.
. Prior management/supervisory experience highly preferred. At least five (5) years of Telemetry RN experience in an acute care setting. At least three (3) years of experience supervising/managing a Telemetry unit.
PACU Nurse Manager
The Medical Center is a Magnet Recognized Hospital and a Level II Trauma Center. 400+ acute care beds and 30+ Emergency beds.
. Bachelor’s degree in Nursing.
. Master’s degree preferred.
. Current Hawaii license as Registered Nurse upon hire.
. Evidence of continuing education.
. Current BLS/HCP certification.
. Minimum five (5) years progressive experience as a Registered Nurse in an acute care facility of which at least three (3) years have been in a leadership capacity demonstrating the ability to manage assigned areas effectively. Previous management experience in a pre/post area is required.

Shift Differential:
$2.00 more per hour 6:00 pm-12:00 am midnight

$1.50 more for each hour worked
