A large health system in the Raleigh, NC area, has a need for a Certified Pediatric Diabetes Educator to join their team. The Pediatric Diabetes and Endocrinology Nurse Educator is responsible for participating in the development, implementation, and evaluation of the Pediatric Diabetes and Endocrinology Program. The position will provide education to pediatric patients and their families in the inpatient and outpatient classroom settings and provide supplemental education for healthcare and school personnel. The Nurse Educator will collaborate with physicians, other program and hospital staff, and school nurses to establish endocrinology management plans and ensure continuity of care. Utilizes a collaborative interdisciplinary approach to anticipate, integrate and coordinate the patient’s plan of care and maximize the ability to remain in a community setting.
The Nurse Educator is responsible for meeting the needs of patients in order to optimize clinical and resource outcomes, and to monitor the effectiveness of the plan and its outcome. In collaboration with the providers, the Nurse Educator identifies at-risk patient populations who require care management services, provides bio-psychosocial assessments to identify patient needs and to assist in the development of the plan; serves as a patient/family advocate and acts as a communication link between provider, patient, family, outpatient and inpatient healthcare team, as well as community resources to reduce redundancy and fragmentation of care.
Experience
Previous experience in pediatrics required. Endocrinology experience and phlebotomy skills (pediatric and adult) preferred. Education Associate’s degree or diploma required from an accredited school of nursing required. BSN preferred.
Licensure/Credentials
Current License as Registered Nurse in State of North Carolina with no substantiated findings required. Certified Diabetes Educator required.
Hours of Work: 40 Hours Per Week
Weekend Requirements: No Weekends
