CASE MANAGEMENT
500+ bed not for profit hospital
Four 10 hr shifts/week, every third weekend
Two weekend positions – Fri, Sat, Sun 7A-7P or Sat, Sun, Mon 7A-7P
Works with patients and medical staff to determine appropriate care
Assess post-hospital care needs, Reports to Lead Case Manager
RN with minimum of 1 year case management experience in acute care
For ER position, clinical experience in ER preferred
Bachelors degree and Case Management certification preferred
Salary range $68-80K + relocation assistance
