Coder openings – Hospital
CCS or RHIT required. Minimum one year experience in coding/abstracting.
This position requires on-site hospital coding experience.
Position Overview:
* Codes and inputs medical diagnoses, and operative procedures in accordance with ICD-9-CM and CPT4/HCPCS classification coding systems. Ensures consistent and accurate coding through thorough review and analyses of diseases, injuries and procedures as written in the medical record.
*Optimizes hospital reimbursement through the selection of the correct principal diagnosis, additional diagnoses and procedures, and groups them into the appropriate Diagnostic Related Group (DRG) or Ambulatory Patient Classifications (APC) in accordance with the Health Care Financing Administration guidelines. Follows appropriate coding compliance guidelines.
*Communicates effectively with physicians regarding documentation requirements and prospective payment determinations which includes reason for admission, treatment plan, coding issues and medical record completion.
*Reviews and checks the medical records of discharged patients for completeness and timeliness. Ensures necessary reports, signatures and other medical information are present and the record conforms to standards established by the Medical Center and medical staff policies, the Joint Commission on Accreditation of Health Care Organizations (JCAHO), the Peer Review Organization (PRO) and other regulatory agencies.
*Performs medical record completion tasks as assigned. Assists physicians and other staff in the medical record completion process.
